The Internal Revenue Service (IRS) Form 8889 is used to report HSA contributions, compute the HSA deduction for federal income tax purposes, report distributions from an HSA, and figure amounts you must include in income and additional tax owed when an individual does not use the account according to tax rules.
Use to file your Taxes
Form 1099-SA (you will get in January) notifies the IRS of distributions made from your HSA duringthe tax year.
Form 5498-SA (you will get in May because you have until April 15th of the current year to make a contribution for the previous year) notifies the IRS of contributions made to your HSA during the tax year.
